Admissions/Transitional Care Nurse

Access Department - Full Time (10:00am-6:30pm)

As a key member of the Access Department RN team, the Admissions/Transitional Care Nurse (ATCN) is responsible for assuring that Hospice of Southern Maine’s programs are effectively represented and accessible to patients, families, referring physicians, and others with an interest in hospice through pre-admission evaluations, informational visits, intake and referral coordination, admissions, other RN assessment visits as needed, and community outreach and education. The ATCN will also help facilitate transitions of care for hospice-eligible patients in hospitals. The ATCN works closely with HSM’s Access Department, Medical Directors, and referring physicians to ensure timely and responsive access to hospice care according to agency mission and goals.

Resource Nurse

Gosnell - 36-Hours (Friday, Saturday and Sunday 10am-10pm)

The Resource Nurse works as part of the clinical management team and is responsible for assisting and coordinating the day-to-day clinical activities at GMHH and assisting with facilitating communication and providing leadership to the GMHH staff. The Resource Nurse is the primary supervisor on site during certain parts of the day. Performs admissions, manages medication ordering and supply, reviews and updates patient care plans and conducts IDT.

Float Nurse II

Gosnell - 36-40 Hours (2) (floating schedule)

The Float Nurse II (RN) is responsible to provide quality, cost effective hospice nursing care, within an interdisciplinary team, in a competent holistic manner for an assigned group of patients, through assessment, planning, implementation and evaluation of nursing care; to empower patients and families to achieve maximum levels of comfort and independence. The Float Nurse II works in the Hospice In-Patient Unit, Hospice Home Program and Access Department depending on the need of the organization. Incumbents will be guaranteed a minimum of 36 hours each week, need to be available to work between the hours of 7am and 7:30pm and at least two weekends per month..
