Make a Referral to Hospice of Southern Maine

To refer somebody to Hospice of Southern Maine so that they may begin receiving hospice services, please do one of the following:

Advance Directive Information

Your advance directive ensures your treatment wishes are known and honored when you are unable to speak for yourself. It helps you outline your healthcare decisions and should be shared with your doctors and loved ones. The best time to complete your advance directive is now - before a crisis occurs. 


Grief Services Information

As part of our mission to serve the entire community, we offer bereavement services—at no cost—to anyone who has experienced the death of a loved one, even if their loved one didn't receive end-of-life care from Hospice of Southern Maine.
