Making Your Wishes Clear Through Advance Directives
People of all ages and conditions of health should take time to document their Advance Directives and discuss their end-of-life wishes with their families. Hospice of Southern Maine and other hospice organizations can provide valuable information to educate individuals about Advance Care Planning.
The Advance Directives form below provided by Maine Health Care may be used to tell your physician and others what medical care you want to receive if you become too sick in the future to tell them what you want. It is suggested that people talk with their lawyer, family members, physicians.
