How do I make a hospice referral?
Hospice provides compassion, care and comfort for patients with a life-limiting illness.
Hospice aims to enhance the quality of life by controlling pain and physical symptoms, maintaining dignity, respecting patient wishes and providing emotional and spiritual support.
Hospice is a family-centered approach that includes a team of doctors, nurses, social workers, counselors and trained volunteers.
To refer your loved one or to make a self-referral, please call the Access Department at (207) 289-3649. Our staff will work with you and your loved one's physician to obtain all of the necessary information.
The following information will be needed:
- Patient name
- Family contact person & phone number
- Facility contact person & phone number
