Home hospice care is a unique model of care where nurses, hospice aides, social workers, faith and spiritual workers, grief counselors, and volunteers come to you wherever you live. Medical supplies, medications, equipment, and other support services are provided.

Hospice supports you and your family with the help you want and need. Our hospice team members visit you wherever you live, even if you have no permanent home. A nurse is always available to answer questions and manage symptoms.

If you or a family member have noticed health changes like more pain, shortness of breath, confusion, or weight loss, hospice care may be right for you. Your health care provider may recommend hospice care, or you may call us to determine if you qualify for hospice services.

If you have health insurance, we will bill your insurance. If you do not have the ability to pay, we provide free services to everyone living in Cumberland and York Counties.

If you aren’t fluent in English and have your own interpreter, please call us directly at 207-289-3649. If you need an interpreter, please contact your health care provider for their assistance with this. If you don’t have a health care provider, please email us at access@hospiceofsouthernmaine.org to arrange for interpretation services.
