Transition to New Physician Model
Hospice of Southern Maine is pleased to announce a new collaborative model for the delivery of physician services. On July 1st , we will begin working with Maine Medical Center’s Palliative care team and New England Cancer Specialists (formerly known as Maine Center for Cancer Medicine) to provide medical services for patients at Hospice of Southern Maine. The new relationship, under the medical direction of hospice and palliative care expert, Dr. Tom Keating, will enhance continuum of care for patients in the greater Portland community and southern Maine. HSM provides compassion, care and comfort through end-of-life to over 1400 patients and their families in their homes, assisted living centers and at our 18-bed Gosnell Memorial Hospice House.
The collaboration of Hospice of Southern Maine with MMC and NECS will enhance the quality of care as patients transition from a hospital setting to hospice care. Benefits will include:
- Cohesive coverage by a consistent team of highly skilled palliative care physicians,
- Timely referrals to hospice will support the greatest quality of life at end-of-life
- As MMC and NECS patients transition to HSM, their physician relationships will continue
The collaboration will also allow for additional opportunities for education and research as the HSM providers will act as clinical instructors for MMC Palliative Care and Geriatrics fellows. Additionally, the role of the Gosnell Memorial Hospice House as a hub for hospice training will expand to include more medical students as well as interns and residents.
