Hospice Referrals
Hospice is quality, compassionate care for patients struggling with a life-limiting illness.
Hospice aims to enhance the quality of life by controlling pain and physical symptoms, maintaining dignity, respecting patient wishes and providing emotional and spiritual support.
It is a family-centered approach that includes a team of doctors, nurses, social workers, counselors and trained volunteers.
To refer your loved to Hospice of Southern Maine, please call the Intake Department at (207) 289-3640. Our staff will work with you and your loved one's physician to obtain all of the necessary information.
- The following information will be needed:
- Patient Name
- Family Contact Person & Phone Number
- Facility Contact Person & Phone Number
- Patient demographic sheet (including name, address, insurance info, etc.)
- History and Physical or discharge summary from a recent hospital stay OR
clinical notes to help give a picture of what is happening clinically with patient (Doctor visit notes, RN notes) - Current Medication List
- Physician order for hospice
- DNR/DNH (if available)
