for both the patient and family. Our interdisciplinary team of healthcare professionals works with the patient's family
and personal physician to ensure that each need is met, and we provide care in nearly any setting.

in a private home, nursing home, or hospital, we are available to help people face the unique challenges of terminal
illness. We can help by offering:
- Physician and nursing care to maximize comfort and minimize
pain
- Medication management for pain and symptom control
- Medical equipment and supplies
- Home health aides to assist with personal care needs between
nursing visits
- Volunteer support to give comfort and assistance
- Education, social work, counseling, and bereavement services
- Pastoral care
- Other specific treatments and therapies as needed
