Making Your Wishes Clear Through
Advance Directives
People of all ages and conditions of health should take time to document their advance directives and discuss their end-of-life wishes with their families. Hospice of Southern Maine and other hospice organizations can provide valuable information to educate individuals about advance care planning.
You can get a Maine Advance Care Planning Packet from Caring Connections, a program of the National Hospice and Palliative Care Organization. Caring Connections tracks state and federal legislation and significant court cases related to end-of-life care to ensure their advance directives are current.
