Coordinated Hospice Care in Your Facility
While many Hospice of Southern Maine patients receive care in their homes, we also provide end of life services in assisted living facilities and long-term care facilities. We believe that it is best for residents who are familiar with their surroundings and the facility staff to remain in a comfortable and secure environment and to allow Hospice of Southern Maine to collaborate and supplement the care of these residents. Hospice can offer expertise in symptom management, offer emotional support to residents and their families, provide volunteers for companionship, and provide chaplains and social workers for emotional and spiritual support. Our goal is to enhance and support the facility-based care team.
This program is available for any Cumberland or York County hospice-eligible patient, in facility, with advanced, end-stage illness. The following guidelines may indicate the need for the program:
- Frequent doctor visits with related complications
- Frequent emergency room visits or hospital admissions in the past year
- Deteriorating functional or physical status, frequent infections, unplanned weight loss
